90-minute intake
$300
A comprehensive psychiatric evaluation with time to talk through what matters most.
30-minute follow-up
$175
Medication management with supportive therapies and practical next steps.
QbCheck ADHD testing
$150
Available at home or in the office.

Shorter and longer visits are available. Call or text the office to ask what may fit your needs.
A small scheduling kindness
For an intake, we are both setting aside 90 minutes to do this well. A little skin in the game helps protect that time and keeps appointments available for people who need them. If you need to cancel or reschedule, please give us more than 23 hours’ notice. Missed intakes and late cancellations are charged the full self-pay rate of $300. Missed follow-up visits and late cancellations are charged the full self-pay rate of $175. We know life happens, just give us a heads-up when you can.

GETTING STARTED
What happens before your first visit
1. Charm Health
Once you contact us, our office will ask for a photo ID and insurance card. We’ll begin your Charm Health intake, including your biopsychosocial assessment, mental health screeners, office policies, and HIPAA paperwork.
2. Headway
You’ll receive a Headway invitation to complete your financial agreement, consents, and card-on-file information.
ONLINE BOOKING
Your appointment is confirmed when your intake is complete
An online appointment is a request—not a confirmed appointment. Your visit is confirmed after Charm Health and Headway requirements are complete and your insurance is verified. Please complete all requested items at least 48 hours before your appointment.

Good Faith Estimates for self-pay patients
If you do not have insurance or are choosing not to use insurance, you have the right to receive a Good Faith Estimate of expected charges before care. We will provide one when required or whenever you request it. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be able to dispute the bill. Call or text our office to request an estimate.
